Bring a large pot of salted water to a boil. Cook the penne pasta according to package directions until al dente. Reserve 1/2 cup of pasta water before draining.
While the water heats, heat olive oil in a large skillet over medium heat. Add the minced garlic and red pepper flakes (if using), sautéing for about 1 minute until fragrant but not browned.
Pour in the crushed tomatoes, salt, and black pepper. Simmer for 10 minutes, stirring occasionally, to allow the flavors to meld and the sauce to thicken slightly.
Reduce the heat to low. Slowly stir in the heavy cream until the sauce turns a uniform pink color.
Stir in the grated Parmesan cheese until melted and smooth.
